Videoconsejo sanitario sobre haloperidol, qué es, y cuándo y cómo debemos tomar este principio activo. Videoconsejo impartido por el farmacéutico Carlos Fernández Moriano, elaborado por el Consejo General de Colegios Farmacéuticos y producido por Medicina TV. El haloperidol es un fármaco antipsicótico del grupo de los llamados clásicos o convencionales, que actúa sobre nuestro cerebro, sobre todo como antagonista de receptores de la dopamina, y regula la actividad de las neuronas de ciertas áreas cerebrales, dando lugar a efectos beneficiosos en determinados pacientes con trastornos psiquiátricos, como la esquizofrenia o el trastorno bipolar. Está autorizado también para el tratamiento de tics, así como de conductas de agresividad que aparecen en ancianos con Alzheimer o en niños y adolescentes a partir de 6 años con problemas del desarrollo o retraso mental. Aunque su uso como antipsicótico ha ido siendo sustituido por otros fármacos y suele usarse en casos en que otros antipsicóticos fallen, en la farmacia comunitaria disponemos de medicamentos con haloperidol en forma de comprimidos 10 mg, de gotas orales en solución 2 mg ml y de solución inyectable de administración intravenosa/intramuscular 5 mg ml. De forma general, se recomienda una dosis inicial baja, que puede ir ajustándose posteriormente de acuerdo con la respuesta del paciente. En adultos, las dosis diarias habitualmente recomendadas suelen ser de 1 a 10 mg, administradas en 1-2 tomas, y no deben superarse los 20 mg diarios. En pacientes de edad avanzada y niños, la dosis máxima es de 5 mg día. Las gotas orales se deben emplear solamente para dosis que no puedan conseguirse con los comprimidos 1-2 mg. Para la toma de éstos, lo más conveniente es tragarlos enteros, sin masticarlos, acompañados de algún líquido, no alcohólico, después de las comidas. No obstante, es importante seguir las dosis y duración del tratamiento fijadas por el médico y las recomendaciones establecidas por los laboratorios en los prospectos, ya que pueden existir diferencias en las dosis dependiendo de la enfermedad para la que se use. Si el paciente olvida una dosis, se recomienda que tome la próxima dosis de la forma habitual; no debe tomar una dosis doble para compensar la olvidada. El uso continuado de haloperidol debe evaluarse periódicamente, al menos cada 6-12 meses, y es aconsejable retirar gradualmente el tratamiento, siempre cuando lo recomiende el médico. Conviene recordar que el haloperidol no puede usarse en pacientes con Parkinson o con antecedentes de ciertos problemas cardiacos. Se han notificado casos de prolongación del intervalo QT del ECG y la aparición de arritmias ventriculares potencialmente graves, por lo que no se debe tomar a la vez que otros medicamentos que prolonguen el intervalo QT, como algunos antibióticos p. ej., azitromicina, claritromicina, o levofloxacino o algunos antidepresivos p. ej., citalopram, escitalopram, entre otros. Entre las principales reacciones adversas del haloperidol, es muy frecuente la aparición de agitación e insomnio puede reducirse evitando la toma antes de acostarse si es una toma única, pero también de movimientos involuntarios temblores, contracturas musculares, etc., alteraciones del peso o dolor de cabeza. Además, podría producirse cierto grado de sedación o deterioro de la capacidad de reacción, efectos que pueden verse potenciados por el alcohol, por lo que se recomienda que los pacientes no conduzcan ni manipulen máquinas durante el tratamiento hasta conocer su susceptibilidad a estos efectos Y recuerda, pregunta siempre a tu farmacéutico de confianza. There has been a considerable rise in antipsychotic medication use in pregnancy, from 3 per 1000 pregnancies in 2001 to 8 per 1000 in 2007. Case reports, case series, voluntary registries, and small cohort studies suggest that antipsychotic drug exposure in pregnancy is unlikely to cause congenital anomalies but may be associated with preterm birth, low birth weight for gestational age, and neonatal withdrawal effects. However, most data were based on the older, “typical” antipsychotic drugs, which are now less commonly used. Second generation antipsychotic drugs were first introduced in the early 1990s and differ from earlier agents in terms of their side effects, especially their potentially higher risk of metabolic syndrome and venous thromboembolism. The few studies that have evaluated atypical antipsychotic drugs in pregnancy were limited in sample size, and in the handling of confounding by indication. Accordingly, these studies may not have been equipped to evaluate the impact of antipsychotic drug use in pregnancy on important maternal medical and perinatal conditions. In this study, the researchers evaluated maternal medical and perinatal outcomes associated with antipsychotic drug use in pregnancy in multiple linked population health administrative databases in the entire province of Ontario, Canada. Methods In this multicenter, blinded, placebo-controlled trial, we randomly assigned adult patients with delirium who had been admitted to the ICU for an acute condition to receive intravenous haloperidol (2.5 mg 3 times daily plus 2.5 mg as needed up to a total maximum daily dose of 20 mg) or placebo. Haloperidol or placebo was administered in the ICU for as long as delirium continued and as needed for recurrences. The primary outcome was the number of days alive and out of the hospital at 90 days after randomization. Results A total of 1000 patients underwent randomization; 510 were assigned to the haloperidol group and 490 to the placebo group. Among these patients, 987 (98.7%) were included in the final analyses (501 in the haloperidol group and 486 in the placebo group). Primary outcome data were available for 963 patients (97.6%). At 90 days, the mean number of days alive and out of the hospital was 35.8 (95% confidence interval [CI], 32.9 to 38.6) in the haloperidol group and 32.9 (95% CI, 29.9 to 35.8) in the placebo group, with an adjusted mean difference of 2.9 days (95% CI, −1.2 to 7.0) (P=0.22). Mortality at 90 days was 36.3% in the haloperidol group and 43.3% in the placebo group (adjusted absolute difference, −6.9 percentage points [95% CI, −13.0 to −0.6]). Serious adverse reactions occurred in 11 patients in the haloperidol group and in 9 patients in the placebo group. Conclusions Among patients in the ICU with delirium, treatment with haloperidol did not lead to a significantly greater number of days alive and out of the hospital at 90 days than placebo. Patients say sedative known as "booty juice" injected against their will A growing number of children and teenagers admitted voluntarily to North Texas for-profit psychiatric hospitals told WFAA that they've been injected with powerful sedative drugs without their parents’ knowledge. The young patients and their parents said staffers at several psychiatric hospitals describe the practice as injecting patients with "booty juice." They claimed the injections often were administered in response to even minor disciplinary reasons. One 16-year-old patient told WFAA that she was injected in her buttock with “booty juice” because she demanded to see a doctor. “They took me in my room and they were like, 'Bend down,'" said the patient, who declined to be identified. “…They put me on a chair and held my arms…(They) put it in my cheek.” She's among dozens of voluntary private psychiatric patients who've told WFAA they've been held against their will after seeking treatment. Dallas Attorney Ramez Shamieh said he’s familiar with the term "booty juice." That’s because he has filed lawsuits against psychiatric hospitals for illegally subduing children. WFAA became aware of the term booty juice in its series “Against Their Will.” During the interviews, various children and teenagers admitted voluntarily to several psychiatric hospitals said they received or were threatened by hospital staffers with booty juice injections. Dr. Ebraheim’s educational animated video illustrates injection methods of the Gluteal region or the buttock. Gluteal injection, buttock injection or Glute injection is common but it can injure the sciatic nerve. INTRAMUSCULAR INJECTION OF THE BUTTOCK SHOULD BE SAFE. Glute (buttock) injection is a universal procedure that is commonly used around the world. The injection has to be safe. Selecting the proper site that avoids injury to the neurovascular structures is important. The sciatic nerve could be at risk for the injury during buttock injection. Buttock is divided into quadrants. Divide the buttock halfway down the middle and halfway across. Injection is given in the center of the upper, outer quadrant. Injection into the other quadrants may cause injury to the neurovascular structures. Another helpful method could confirm a safe site for injection is to draw a line from the posterior superior edge of the iliac spine to the greater trochanter. The area above the line is considered to be the safe zone for intramuscular injection. This corresponds with the upper outer quadrant of the buttock and is the exact safe site for the buttock injection.
